Licensed Clinical Social Worker - Case Management

Position Overview:

The Social Worker is responsible for providing appropriate social services to patients, families, and others identified as having social, psychological, and/or environmental needs related to their rehabilitation admission or adjustment to their rehabilitation or disease process. The Social Worker is responsible for the evaluation of patients; development, coordination, and implementation of the treatment plan; maintenance of informative, accurate, and timely documentation; participation in team meetings; training of others as indicated; and participation in continuing education.

Essential Functions:

a) Evaluates patients.

b) Develops treatment plans and goals.

c) Implements treatment plan.

d) Completes documentation.

e) Participates in team meetings.

f) Provides training and learning opportunities as assigned.

g) Participates in continuing education opportunities

Physical Demands and Working Conditions:

While performing the duties of this job, the employee is regularly required to see, talk and hear. The employee frequently is required to sit and use hands along with fingers, to handle or feel. The employee is occasionally required to stand, walk, reach with hands and arms, climb or balance, and stoop, kneel, crouch or crawl.

Qualifications & Skills:

a) Education: MSW with 1-2 years of experience in rehabilitation social services or BSW.

b) Training & Experience: 1-2 years of rehabilitation social services experience is preferred.
